Six steps from enrollment to a closed PCP loop.

1. 01

   Enroll

   Your care team or API enrolls a patient into a Loopnora program with the minimum chart packet we need to practice safely.

2. 02

   Reconcile

   We assemble meds, allergies, problems, and recent data — flagging conflicts before anyone touches the regimen.

3. 03

   Clinical visit

   An Advanced Pharmacist Practitioner completes the encounter under California standard of care and Loopnora protocols.

4. 04

   Change the plan

   When appropriate, we initiate, adjust, or discontinue therapy — documented in a structured med-change table.

5. 05

   Close the loop

   We notify the PCP or diagnosing prescriber (or write to a shared record) and log delivery.

6. 06

   Follow through

   Scheduled follow-ups and escalations keep momentum between physician visits — without replacing them.
