Reconcile
We assemble meds, allergies, problems, and recent data — flagging conflicts before anyone touches the regimen.

Clinical visit
An Advanced Pharmacist Practitioner completes the encounter under California standard of care and Loopnora protocols.
- 04
Change the plan
When appropriate, we initiate, adjust, or discontinue therapy — documented in a structured med-change table.
- 05
Close the loop
We notify the PCP or diagnosing prescriber (or write to a shared record) and log delivery.
- 06
Follow through
Scheduled follow-ups and escalations keep momentum between physician visits — without replacing them.
